The problem with not getting exhaust out from under the truck, goes beyond sucking in fumes. The exhaust mixes with moisture and become acid. That acid eats everything it comes into contact with. It doesn't matter if it is plastic, rubber or steel it eats it very slowly. Stainless is about the only thing that it doesn't seem to bother.

Your choice, your health. If you don't smell exhaust fumes please don't think that carbon monoxide doesn't exist in your cab
, when i start my truck my bulldog always races for the tail pipe no matter where he is in the yard, had my truck idling for 10 mins one day looking at the engine and he was barking and snapping right at the outlet, this went on for the whole duration i was at the front, walk past him to enter the shed and turned around to tell him to shut the :censor:up and caught him staggering to both side of the vehicle (know think of charlie chaplin dancing sideways) flopped over on his side tongue hanging out drooling and not moving, picked him up and took him to fresh air and started CPR (know think of Mr Bean giving CPR with his foot) was not giving him mouth to mouth the slobbering bastard. came good after 20 mins, but still does the same thing every time.

Out the side, as originally designed.
 
I just replaced exhaust manifold gasket, and the collector gaskets. I am pretty confident I have no exhaust leaks. Are you running without a top, no doors, and bikini top only? I am sure there are different variances with whatever top/door combo you have which will create different venturi affects. It makes sense having it come out the side, push the fumes outward to get away from the suction area of it being vacuumed back into the cab.
Run both topless and with the hard top on. My exhaust doesn't go straight back but at a 45* between the rear tire and back sill.
 
New exhaust installed, 100% yes a better location. Drove it yesterday and never once smelled exhaust. Thanks for all the feedback! And the Dynomax UltaFlow welded is a great sounding muffler, perfect for what I was looking for.
