CT20 Water Cooling Flange - Inlet Outlet on same flange?

Hi,
While searching on the internet for part identification for CT20 Turbo Water / Coolant Flange I came across these designs. Found in Amazon this. Found it strange that the Inlet and Outlet are from the same flange. Couldn't find an actual picture of a CT20 turbo water cooled installed.
Am after your views. Tks

Was reading Garret website for turbo cooling. Which used two separate ports for in and out.
