What the difference between the two case transferbox of an FJ40 PTO Winch. Why the second gear-wheel in the first image? Thanks.

I've got both on the shelf and haven't messed much with them yet but the below is Engaged, disengaged. Use trans to go forward and back with reverse.
Other is a i believe reversible PTO, can go forwards or back with the block off plate you see on early FJ40's.. (well don't see them much anymore) Top one looks to be FJ40 period prior to split case.. Lower one is later split case pto
 
Yes single direct and shift lever is on the front verses the back. There are two direction with the lever in the front and back for split case. I do believe the single direction the winch runs the opposite direction. The worm gear in the winch is revesed in the winch for these. Not positive on that. Still planning on converting a single direct split case PTO to a two direction using a parts from a earlier PTO from a one piece transfer case.
 
not sure the didderence but some have a reverse idler gear and some dont acording to the fsm
