92 w/ 178000. Whenever starting the truck it feels like I have a broken engine mount. The truck shakes and vibrates badly while cranking over then smooth once it starts. Any ideas?

yes they brake, I busted one myself, but to be honest it drove fine(for over a year) with no signs of a busted mount........finaly noticed it was broken.....

But to test, it best to have a helper. Put in gear, set parking brake, mash down on brake pedal, then give it some gas, look at the driverside mount to see it its seperated. If busted you will also notice the motor moving to the passangerside but not the body. Do the same in reverse and look at the passangerside mount(but its less likely to go brake)

You can also do the same test by yourself, just open the hood and you can see enough of the motor to see if it moves or not. Also the t-case lever will move 1-2" to the right if there is a busted mount. Can even just to the same test with the hood closed, if the body moves then the mount is prolly OK, it it doesnt move then it prolly busted.

to be honest it doesn't sound like a broken mount to me, not sure what however,

I test drove a 91 with a broken mount (dealer said the part was on order), the symptoms where not similar to what you're describing.
