Brake pad delaminating

I just found out the brake pads on 3 out of 4 brakes are delaminating from the steel backing plate. They are fairly new with over 75% of the pad remaining. Theses are OE that I have used/replaced since my 2006 LC hundy was new. I can only think that the added weight of all my mods has stressed them to the point of failure. It could also be the way I drive that is a "little" hard on the brakes. Does anyone know if this is a common problem with heavy mod rigs? Is there a better or heavy duty brake pad that should be used or is it just a freak accident that I got three bad brake pads. Luckily, I found out while I was getting new tires and it was noticed. If theses had failed on the road or on the trail it could have been an expensive and possibly life threatening disaster.
 
Did you have your original pads while having all your mods and did they fail?

I ask as the pads that came on your truck are not the same pads that you buy from Toyota when you have your brakes replaced.

OE pads are Akebono ceramic I believe.
