There is a thread on this. I had the same problem. Need to clean drain and fix the seal using some sealant or gasket material. Use the search.

I have seen many with the same issue I have on my 2003 LX470. The FJ100 Land Cruiser will have exactly the same setup, of course. I had water dripping on the passenger floorboard.. Posts inidicated it was probably the condensate tube plugged. But I couldn't find anything that showed how to find the exit on the firewall. I found it and to make it easier for others, I am posting pictures. These are all from the engine compartment. The black rubber drain end is a little hard to see, so in the one picture, I inserted a Phillips screwdriver into the tube so you can't miss the location.

I used a long twist-tie (like from a bread bag) to push up into the drain fitting and dislodge whatever was blocking it. At first, I thought it wasn't going to work, but then, the twist tie went much farther up inside. I was also concerned about this crud/mold/smell issue. I had no smell, but I thought I would do preventive maintenance while I was at it. I had just opened the cabin air filter door, so I had easy access to see inside. My evaporator looked clean, but I sprayed it with one of the foaming coil cleaners that come in a spray can. My coils and fins looked nice and clean. Afterwards, I squirted in some water. This whole process was easy - and I didn't have to drill any holes in the houseing. Water now drains freely from my condensate drain exit on the firewall.

Like others, I intend to put a little hose extension on to this rubber tube. I think it will make the drain less likely to plug and it will route the condensed water away from dripping on the frame, etc.
