40 Front Fender Rust and Painting - Noobie Question

Rust Question:

Ok, so I am a newbie to body work. I have searched and didn't find what I was after. I am looking for options on fixing this:

It is the worst of the rust I am up against on my Feb 78' 40. I hit it with a wire wheel to get some of the surface rust off and this is what was below. I am assuming a cut and weld in patch is the way to go on something like this? Being that I don't have access to a welder I was wondering if just buying a fender was cheaper than having a shop fix something like this? There is a little surface rust on the inside of the engine bay as well. I figured I would try and fix it all at once.

Painting question:
How smooth of a surface do I have to get before I apply primer? There are small raised areas around the 40 that I can feel when I run my hand over them. There were multiple coats and spots of spray paint added by the PO. I have never done body work before so this is all new to me. I divided the hood into a few tests.

I took the advice of one group and used stripper to see if I could get it to bare metal. It got most of the old paint off, but there are raised spots now between the paint patches and bare metal. On the far side of the hood I wet sanded the paint with some 600 grit. It feels way smoother, but there are little spots under spray paint that I can feel are divots. Is it better to have the bare metal or some paint under when I apply the primer?

Thanks in advance, I know this is some basic stuff, but its all new to me.
