I purchased a 2010 LX with 125k miles about 8000 miles ago, and it had been meticulously maintained at the Lexus dealership. The only thing I had noticed is an odd rumble/hum noise at 2000 rpm's. I could feel it in my hands and feet. It's not a noticeable noise. It's more like transfer on my hands and feet.

Bingo!

Had the front shocks replaced (both leaking) and that took care of the problem (i hope). During the replacement, everything that could go wrong or break did, strut tops, link assy, support sub-assy, cushion shocks...also found a lousy caliper, so they replaced the front rotors and breaks, and the one busted caliper. Three weeks to get all of the parts and repairs finished, thank god Toyota of Bellevue provided me with a brand new Camry to drive... Merry Christmas to me ($2000)

Glad they figured it out and you are better than whole.

There was a TSB with these exact symptoms that effected 08-09 LC and LXs. Possibly early '10s too. A stackup of tolerances could contribute to this as very few examples actually had this issue.

Any of those parts you listed, shocks, caliper, etc - could definitely also contribute.

Pretty good price on the ahc fluid. I just paid like 28 liter at lynnwood

Got excited before a good test drive!

Unfortunately after driving over a couple of mountain passes yesterday the vibration problem hasn't been solved...

One thing to try is to grease the front and rear drive shafts.

One very simple, cheap thing to rule out -

Common issue on the 200 with high miles is the blower fan on the HVAC. The bushing wears out and the fan vibrates, often to the point of feeling it in the steering wheel. There can be a soft rumbling noise.

Next time you feel the vibration, try turning off the HVAC fan to see if the vibration goes away.

If you're lucky enough that this is your problem, there's a thread here on Mud that tells how to fix it.

Fingers crossed! I did two rounds of transmission fluid drain and fill (6.5 quarts), and I think it cured the problem. I'm heading up a mountain on Sunday, and I'm praying my problem is solved! Go figure = During the drain and fill, my radiator cracked in the usual spot and had to have it replaced (Seattle Toyota $1000 could have had it done cheaper, but I was desperate to get my car back by the weekend).

The transmission drain and fill solved most of the problem; the vibration is 90% cured. I'm going to do two more drains and fill this week and see if we are as good as new.

Wonder if the fluid level was low prior to flushing?

I have an ‘08 that has had a hum since new. I believe that it is a resonance that is felt in the steering wheel and is harmless. Go ahead and try all the suggestions and get back to me. 200,000+ miles and the resonance is the same as new and occurs at the same rpm as it did when I drove it off the lot. If it really bothers you don’t drive at the rpm that hums.

Drain and filled a total of 14qrts of ATF WS fluid and it made a huge difference, the shifting is much smoother and my problem is solved...

Great news!

My ongoing problem is still haunting me!
Self-diagnosis number 21: The original owner of my LX purchased it new with 22-in rims and tires (125k with the 22's), any chance that the oversized wheel's could cause the front wheel bearings to go?

125k could make the front wheel bearings go. Maybe some water got in? The wheel bearings are not particularly hard to replace, you can buy new hubs with the bearings already installed. It is four more bolts and the axle nut past a brake rotor replacement.

Just ordered my front wheel bearings, I will keep you updated on the outcome. Has anyone done a DIY video of the replacement? What size is the large socket need? Thanks

TLDR: Full flush and filter change resolved symptoms of occasional harsh shifts and slow engagement between PRND positions.

Data point for posterity -- on a 2011 LX570 with 85,000 miles, I did a single 2.7 qt drain/fill prior to a long trip with a 6000 lb. trailer. Shifting during trip was occasionally excessively firm, and frequently slow to engage when shifting among Park, Drive and Reverse. Service history unknown. Cleaned pan, changed filter and flushed 14+ quarts of Toyota WS fluid into filler and out through the cooler return line. The old fluid was dark, as expected. Symptoms were resolved.
