Similar problem - TRAC Off, Check Engine Light (CEL) and a flashing 4LO.
Read everything here, decided to check the gas cap first, took it off, put it back on with a secure couple clicks, started it up and it was cleared.
I'll check the codes with my reader later and report back if anything...

VERY interesting.
I tested today with my Samsung Note 10 - almost exactly the same results.
My only difference was I could go into reverse, see the Notification, go to Park or Drive then back to Reverse and it would be gone.

You know what, I have a newish, unused Apple phone I could try. Here's the crappy rub, I also have five sons and they fight each other when they borrow my car to add themselves (remove their brothers) from the VERY limited number of Bluetooth slots in the LC (stupid limit - 4- really!).

But, while convalescing, if you want to look around, my guess is it has something to do with all our Samsung phones updating Bluetooth.
Our 'old' LC's don't have updates like that.

That red box notification appears only when I go into reverse and stays there.
I was asking if anybody else had the same problem - is it a setting I accidentally turned on.
No idea, want it to go away.

- Happens at start and Reverse just as driving for a while then going into Revers

This is a strange one but, looking at this picture, first, no, I haven't found a magic button combination that allows me to look into bellies to use the standard Toyota Ultrasound.

I think this chart, at least for me particularly, is also a guide to the visual changes - for example, the hood 2008 is very different then the hood of the 2011.
