Would bad upper ball joints make a "clunk" sound?

Yeah, but that's usually more from reverse to drive or park to drive, correct? I have that as well, greasing the driveshaft helped, but have excessive slop in the final drives, so, regearing is in the crystal ball! Plus then can correct for larger tires...
 
So, if you can get the rig up off the ground, check for slop in the rack by moving tires side to side, hands at 3 & 9. Also have someone wiggle the steering wheel just a little bit, and see if the rack is moving. Bushings may be faulty, even if the rack is in good shape mechanically.
I didn't find out the rack was bad in mine till after replacing the old worn out shocks, cause I couldn't hear it flopping around. The sway bar bushings would be another good thing to replace, since they get hammered and shrink.
 
Potential front end clunks:
-Blown shocks (no charge)
-Blown lower shock bushings (may appear fine, but shot)
-Sway bar bushings
-Front diff mount
-Driveshaft (over bumps)
-CV drive plates (R to D clunk)
 
I did the 3 and 9 thing after locking the steering wheel, but wasn't sure how much movement there should be. I also didn't have a helper so I wasn't sure if the steering wheel was wiggling back and forth too. I'll try that again next time I've got it off the ground.
 
I had a similar clunk, turned out to be sway bar links. If it's just a clunk you''re in the neighborhood, just need to find out which part it is..
 
I bought the Monroe shocks yesterday, I can't wait to install them, might not be till Sunday though. I'll add an update whenever I install them.
 
Got busy tonight and swapped the shocks. The bottom sleeve on the old shocks fell out of the bushing on both sides once removed. I haven't taken it over speed bumps, but I did drive it around a bit, bumped up over a few curbs, and it feels a lot better, no knocking sound.

Edit: More speed bumps this morning, no sound at all. I believe 87warrior hit the nail on the head with his guess (knocking sound was base of shock hitting lower shock mount).

I have a very similar clunk, done all the maintenance mentioned by Mr Snappy including the new steering rack bushings (went aftermarket), and still a clunk. Mine comes when I brake hard going forward, like the slop from somewhere in the front suspension is moving forward to a sudden stop! It must be something shifting back and fourth because this only happens soon after I reverse, and brake hard the first time moving forward. It won't repeat again until I reverse again and brake hard moving forward. The only other maintenance I haven't done are the front diff bushings, open to any suggestions or ideas....
