Transmission won't shift until 2800 rpm

and if I'm not giving it 30% or so throttle at 2800 rpm it's a hard shift with a loud bang. Any ideas? It's a '92 with 216k as far as I know never been opened up.

Few thoughts, in order of easist to check:

1) Tranny overfilled.

2) Kickdown cable out of adjustment.

3) Filter screen clogged.
