When removing the factory roof rack, the rear passenger side mount came loose inside the frame. Has anyone experienced this and/or have a recommendation on how to access that area to fix? The remaining points have the hardware still in tact. I thought about using nutserts, if absolutely necessary, but would prefer to fix it properly. TIA

Very common problem on virtually all Toyotas. That's why it's recommended to loosen/tighten only a few revolutions per bolt before doing the same thing to the other in the pair. Keep them "balanced", so to speak, and that helps prevent the nut/plate from dropping.

The proper fix is to drop the headliner, find the loose nut, re-attach it.

This is what I did to install an LC rack on my LX and what will be doing with the new rack.

I am about to install a Gamiviti rack, so this is a timely post - it inspired me to check out amazon for something easy I can buy from home:

5x ~9" M8-1/25 All-thread rods. Just ordered them and will plan to use them for my install. I also bought a sheet of 3/16" neoprene rubber to put under the feet:

To the OP's question - I have heard that you can try to 'fish' the mounts back out and that that approach is less invasive than dropping the headliner. You can use a bolt (or maybe all-thread?) or even a coathanger (maybe a dental pick type tool or one of the earwax camera probe jobbies?) My research suggested that it is easier to not lose the mounts in the first place. Sorry for your bad luck!

This same thing happened to me and I have those holes covered with silicone until I get around to either taking the headliner off or getting another threaded setup in there. It sucks, but at least my interior is staying dry. Good luck
