The factory PTO winches allow the cable spool to free-wheel by unlocking the drive. Thats great for manually pulling out the cable to attach to the anchor. Then you lock the drive and winch the cable in for a forward extraction.

Question: Can you spool out the cable under power?

For example, lets say you are on a very steep hill and the only way to descend is to back down using the winch. Or perhaps a climber has fallen into a crevasse and you need to lower a rescuer with the winch cable.

Is it as simple as putting the trans in reverse and THEN engaging the PTO transfer gear?

Yes

I would engage the PTO first (t-case in N) and then use the transmission gears as appropriate to move the winch. 1-5 is for in, R is for out
 
Or get one of the in - out PTO boxes, then you can use any gear you like in either direction.
