I'm running Cooper A/TW 265/70-18s. They fit with stock suspension, I have the F-Sport wheels, not sure if they have the same or a different offset from the normal OEM wheels. I only have some very slight rubbing. I did have to remove the front mud flaps and the wind flaps in front of the tires under the bumper. No other modifications. They are listed at 32.64" in diameter when new.

Would 265/65/18 KO2s fit on a stock gx460 without any rubbing issues?

I assume "stock" means OEM wheels with no suspension lift. Yes, 265/65-18 fits perfectly ... in fact this size fills out the wheel well perfectly with absolutely no rubbing under any condition. Lexus should have gone with this size in the first place. I just purchased a 2019 GX460 and drove directly from the dealer to the tire store and traded for this exact size.

Answer is "Yes, they fit with no rubbing." Nice ride. Slight increase in road noise from stock Pirelli highway tread.

If one doesn't wind up do a lift and need larger tires, that is a darn nice looking setup with that size tire!
