My 2004 LX470 has a factory installed round trailer light connector and I can’t seem to get a parts store round to flat 4 adapter to produce power through it.
Does the Toyota/Lexus require a special adapter?

I use "parts store" round to flat (7-way to 4-way) adapters with no problem on my '06. So should be normal.
 
I think the lid/flap over the round connector on the 100 has the wiring diagram on the inside.
 
The adapters do not produce power, they only transmit it. The same power that goes in, comes out (more or less, what you lose, you can't measure without really sensitive equipment). The circuit conducts plenty of power to illuminate lights on a trailer.
 
The factory plug tests good. I guess I try another adapter and see if the supplies power to the trailer harness

Spray some contact cleaner on both sides - the frame mounted connector and your 7-4 pin adapter and try it again. If that doesn't work, make sure your adapter has continuity from one side to the other, with a multimeter.
 
At times the plug in the truck is working fine but the trailer lights don't work. Often the tabs in the vehicle side need to be bent back into position slightly to make good contact with the plug which in your case is a common adapter.
 
Got it working! Ohmed the round to flat and it was good but the connections were corroded. Also found the ground tabs in the factory receptacle bent out so I corrected that and sprayed with electrical cleaner.
