Lexus/Toyota underwritten Extended warranty/VSA providers?

I'm considering purchasing the extended warranty underwritten by Lexus/Toyota. Anyone have any contact from Lexus/Toyota who offer them as a reasonable price nationally?

Also is the time component decided from in-service date or warranty purchase date?
