Issues with wheel weighs staying on LX570 factory rims?

What’s up everyone, was dealing with a vibration issue before, kinda rough ride over expansion cracks, vibration in floor board, seat, steering wheel, went through and had a road force balance done, felt much better after that, then I set everything back to factory heights and calibrated with techstream, got an alignment, truck was driving great, suspension felt good, everything good, wife the other day asked if I messed with anythjng cause it feels bumpy again and pulls left? Nope, went on a good drive today on roads I don’t typically drive, about an hour and a half highway, truck certainly feels the bumps, vibration is back, I took a look at the wheels and it looks like multiple wheels lost the weights? A bunch of fresh looking adhesive on 3 wheels, 2 wheels don’t have outer weights any more and a couple just remain on the inner side. I’m assuming some of these weights fell off? Has anyone had issues? I’m going to call the place that did it, it was an AAA garage as they were the only road force shop locally, hoping they will redo it. Has anyone had better luck with Lexus doing a balance on the wheel? Definitely seems to be rotational vibration related to this. Curious as to anyone else’s experience. Thanks!
 
I have 13 lx 137k miles. It came with Michelins latitude tires. I get them balanced and rotated
At Costco. They put weights on and they have stuck on. No issues. Costco charges $29.99 for rotation and balance.

Very interesting, I do a Costco run about once a week, is that 29.99 a wheel or total?
 
That is total. I set up appointment online and they usually have rotate and balance within 45 minutes.
 
This is not a common problem, so either they didn’t clean the contact area enough or something is wrong with the adhesive on the weights they use.

With a clean painted surface and good adhesive there is no reason they should be coming off.

Yea, I figured as much, I guess I should have been suspicious when I watched the kid finishing up putting the wheels back on. I specified they have to be hand torqued to 93ft pounds in a star patter, I saw someone have to go show him what a torque wrench was and how to set it. So I double checked when I got home and the lugs were all wildly tight, had to redo them all at home. So hard to find anyone to do decent work in CT.
