Thanks to Marshall and Alan, my front bearings/hubs are snug again and hopefully I won't lose a wheel going to work anytime soon.
On the driver's side, it turns out that the clutch inside the hub had come loose and was probably in the locked position 100% of the time resulting in one side of the axle locked and the other free. It's not supposed to come out of the hub in two pieces...
There are two springs inside the outer hub. One causes the clutch to engage the axle and the other hold the clutch to the cover. The latter was the culprit. It had come out of the clutch. If you look inside the notch, you can see this spring installed correctly.
This is the way it's supposed to look like when you remove it from the hub:
