Harmonic balancer/Timing cover explosion.

I’m an experienced mechanic, I’ve worked on every Toyota they make, this was supposed to be a simple seal/gasket change and it all went to ****. I’m working on a 1988 FJ62 241 strait 6, I replaced the front crank seal and timing cover gasket, started the vehicle and in less than 3 minutes oil was pouring out of the crank seal. I tore it back down and was amazed to find the input shaft on the harmonic balancer had been cut nearly in half and the timing cover around the input shaft had been bent out of shape. I checked for foreign objects; bolts, nuts etc and found nothing just the steel from the balancer. Does anyone out there have any idea, could I have over torqued the crank nut, is there a spacer between the balancer and the crank, or does satan live in this particular FJ? PLEASE HELP I don’t want to put this thing back together only to have the same thing happen. :bang:
 
Did you torque down the timing cover before you torqued down the balancer?
Also, could you please post a pic of the carnage?
