Front end grinding sound

Last December I was in an accident in our 100. It got towed to a reputable collision center here in Phoenix-- I believe it is tied to a local Toyota dealership as well.

Well, after having it back for a few months a loud grinding sound started coming from the driver's side front corner. I took the tire off and couldn't see anything. It was so loud that people walking down the street would watch me drive by. The place I took it to guarantees their work for as long as I own the vehicle. In fact, they state that if I move and something goes wrong I must take it to a Toyota dealership.

I called them up and they towed the vehicle back. They called me and said they loosened up the front end and tightened it back up. Sound was gone.

Fast forward another couple months and the grinding was back. I called again and they had me drive it in. I waited there and the mechanic took it back. The manager came back and told me the wheel bearings were loose and that is what happens as one drives a vehicle. I didn't like the answer but figured it was fixed.

Well it's back, loud as ever. I thought I would get your opinions before I called them back. Not only can you hear the sound but you can feel it in your feet. Mostly in the driver's side but also passenger's side. What could be causing this? Do wheel bearings really go loose? I replaced the bearings on my 60. If I recall you tighten them, then back off a little. Then there was a tab on a nut that you bend. Does the 100 have this also?

I guess this is why one fights to have the car totaled!
 
100's basically the same with the bearings. There's no way they should get loose that often and that quickly (unless they didn't use the star washer). Nor would a loose bearing make that noise necessarily. I'd have them check the CV's and drive flanges.
 
Did they check the bearings for wear? Loose bearings don't make much noise, but worn/dry bearings get quite a grumble.

If it's 200k miles on original bearings (quite possible) then it could just be coincidence, and not accident related.
