what od you mean side cover?
 
is it a 60 or a 62?
 
im sorry i just cant think of anything on that side with a side cover.someone wiill be by who might think differently as of now im late. but ill look on my way out to see if i can think of what your talking about
 
From your description, it sounds bad - or at least you think its bad.. If my engine were making a noise like that I might: Drain the oil and look for metal particles or remove the distributor cap and look for metal particles or remove the oil pan and look at the cam lobes for deep grooves or inspect the rod bearings for excessive play. I'm not an expert with engine noises but you might try a mechanics stethiscope or at least the "rubber hose in the ear" test to localize the noise before you take something apart that prevents you from running the engine. It's easy to know you have a "rumble" but harder to know where it's coming from.
It might be from the side cover as you say, which could be something bad internal. But, it's hard to identify where noises come from. Use a rubber tube/mech stethascope to make sure you know where the noise is coming from. A rumble is usually a bearing. Check the alt, the waterpump, the air pump, etc. Post back with what you find.
 
Behind the side cover are the push rods that push the rocker arms that open and close your intake and exhaust valves. The push rods are pushed by the lifters which are moved up and down by the cam shaft. Got it?

So if you have some rumbling noise behind there, it is probably a serious issue and you need to tend to it immediately. It will only get worse and cause even more damage to your engine. My guess is that it is a bent push rod. These can be replaced without pulling the engine, its not a huge deal, but I think you would need to replace the lifter as well, so both the push rod and lifter are new. Then you will need to adjust your valves and probably set the timing again. You may want to try adjusting your valves and setting the timing first before tearing into things, to see whether this gets rid of the rumbling sound.

Noise like this is probably signaling that its time for a rebuild. How many miles do you have on the truck?
