I can hear the CDL motor running in both directions, but I don't get a CDL engaged light on the dash, even when the key is first turned on and the bottom row of light come on for a few seconds.
Should it come on briefly at" ignition on"?

If not, then could it be that it is not fully engaging? This is a vehicle that has never been in low range, and very likely never had the CDL engaged in it's 16 year life.

Are the switches on the t-fer case a known issue? (wires to it are OK and the plug is in tight).

Did you get this resolved? You can take the TX case switch out and test the continuity across it. Test it a few times, if you don't get a solid reading every time, change it out. While you're under there, its worth using some electrical cleaner to clean up the contacts for both the switch and the motor. remember to re-lube them afterwards with some dielectric grease.
