As long as all the bolts are off and the caliper is removed the rotor should slide off, you could blast the back of it with brake cleaner/PB Blaster and let it sit a while before trying again.
Was just doing a quick look at a Haynes manual.. front rotor on T100 looks to be bolted onto the hub assembly with 6 bolts it appears from the backside.
At any rate I think the hub assembly needs to be removed as if you were replacing or repacking wheel bearings and once the assembly is off then you can unbolt the rotor from it. Not as easy as a 4runner or Taco and just pulling it off now!
I would look at the top of the 95+ section to find the 96 T100 manual link to download on the exact procedure. Section SA 15 2 wheel drive and SA22 4wheel drive. About 2/3rds of the way down in the manual.
