You could probably take that top hinge off and straighten it if it is bent. It sucks though, that top hinge is the absolute worst thing I have ever had to work on.
 
Update, for those who care...

The problem seemed to be the b pillar was canted towards the firewall, I checked both the A and B pillar, the A was nice and square, the B a good tilt towards the firewall.

I unbolted the body mounts starting from the cowl back, then I took the Hi-Lift and spread the b pillar back where the striker plate goes, the metal deformed a bit, but nothing that i'm too bothered about..no trailer rig here. I left the jack in for a few hours.
I then took a good 1/8 shim and placed it between the body mount at the frame under the cowl. Then I snugged all the body bolts down.

Then I loosened the lower cowl side of the hinge and I shut the door. When the door was shut I snugged the cowl side hinge bolts up. Lastly, I did shave down the striker just a bit.

The door shuts nicely now.
