We have a 2013 Lexus LX570 that we purchased L-certified (2017) and added the 5 year platinum Lexus unlimited mileage warranty. (7 years of total extended warranty that expires at the end of October)

Our steering column motor failed and I took it into the dealer for warranty replacement. (which has been done) After repair they noticed what they've diagnosed as the passenger side lifter noise? (bank 2?)

Background

At 157,000 miles (~20 months ago) the lifter repair was performed under warranty. (Noise not as bad then and on Bank 1? driver side) They replaced both sides at that time. I paid for extra parts to be replaced during the repair. (Timing Chains, timing tensioners, dampers, slippers, valve seals) All lifters/adjusters were replaced on both sides. I did not replace any valve springs.

Ask / Questions

Looking for recommendations/advice on what could cause this a second time? Should I replace the valve springs this time around? On the attached former repair order, both timing chains listed on the invoice are the same part number. Are bank 1 and bank 2 timing chains identical or are they supposed to be different part numbers? Any other possible causes of the noise in the video?

Any additional work that you recommend doing while they are in there / disassembled this time? For this to happen again, I'm a little worried there could be something more serious going on with the engine that should be addressed. (We have 188,000 miles now and are nearly out of warranty) Is is it easy to put a new alternator, starter, and possibly radiator on now while the engine is having this performed?

Strange to go bad again in only 30K miles. As you replaced basically all stuff that I can think of and you do not mention any overheating or other engine traumatic event, the only cause left in my book is oil pressure or valve springs problems. AFAIK our valve lifters are hydraulic so there is no adjustment. But problems with oil pressure to the valve terrain can cause issues. I assume you are running the usual oil viscosity 0W20 or 5W30, right?

You can replace starter and alternator, but they are not really in the same location so expect to pay extra for labor.
Yes, radiator is a must do (assuming it is the original). Also add the water pump, belt, tensioner, etc.

I do not see you mentioning the radiator being replaced. If not, make sure you do that. Also add the water pump, belt, tensioner, etc.
Thank You, I have used 0W20 (Toyota/Lexus) since we've owned it. The shop foreman was surprised. (He was the mechanic at the time that did the work and is very skilled) He mentioned the possibility that oil is having a tough time getting up to the lifters that possibly could have shortened their life. (if it's not just a bad lifter that was installed previously)

It's my wife's vehicle and I do not drive a lot, but I did notice that oil pressure was dropping intermittently at idle. (stop light, especially at a slight incline/hill) I'm having them price the radiator. Roughly what should the Toyota radiator part/assembly cost? Will do the water pump and alternator for sure.

Is the oil pump part of the timing cover or can it be replaced separately? I've wondered if the oil pump is getting weak?
 
If you are talking about the oil indicator in the dash, that is normal. If you search, you'll see many people asking about that. I never figured out how that dial works, and in my truck it varies but it is below middle most of the time.

Radiator replacement cost depends on location. I did it myself so could not tell. If you want to buy the parts, there is a sale going on for Lexus parts. I do not know if your dealer will agree to install your parts, mostly do not as they make money from parts to, and justify that by the 12mo warranty. Honestly, I think that is BS as the warranty comes from Toyota anyway for dealer installed Toyota parts. But probably for you now it is more important to have a good relation with the dealer as you want the big expense to be covered by the extended warranty and the work to be done right.

I never had to deal with the oil pump on this engine so cannot tell. Did on other Toyota and non-T engines and all have it on the crankshaft so different location than the valve job. But I suspect that the timing covers have to come off to access the oil pump so this is a good time to inspect.
