Goodmorning, I need to replace the hose that goes from the backside of the throttle body on my wife's 03 gx470. I believe it's the pcm valve hose, but am struggling to find the part number. It's the foam cased hose in the picture( if you look close it has 2 ziptues in the end, I had to stop the leak!). If anyone knows the part number or can point me in the right direction that would be great!

Thank you! Do you have a diagram you used to find it?

No diagram, it's under a category "Ventilation Hose" and it's a Positive Crankcase Ventilation...hose...

There seem to be 3 different part numbers here, one on the left side attached to the intake and one on the right side, that goes to the PCV valve.

See here: Ventilation hose for Lexus GX470 UZJ120L - Genuine parts

12261-50060 is for the left side attached to the intake housing. (see in picture from OP)

The one on the right that goes from the manifold to the PCV valve is 12261-50050 on a 2003. (12261-50070 for 2005+)

The seem very similar but are clearly shaped different.

Also - be VERY careful replacing the one on the right that goes from the manifold to the PCV - several people have broken the plastic nipple on the manifold and it requires replacing the the entire manifold (stupid design).

Thank you for the very in depth answer and advice! I'm ordering currently!
