Typically, the M-12000 is mounted foot forward and the motor is on the right side (looking it it) (driver side). Looks like you might have it backwards in the pictures. Also, my buddy has the same bumper on his Dodge and has to modify it to fit an M-12000 I gave him.

Like mentioned above, bad ground or one of the bulbs are bad. What you describe is normal, don't over think it.
