Transmission output shaft damage

A broken idler gear tooth lead me to pull the stock transmission and transfer case. After seperating the two I discovered the wear on the output shaft. What is your opinion on what would cause so much damage/wear? I don't want this to re-occur if I need to fix something else.

Yea had a lot of slop in it. After buying a used combo, I tightened the crush nut on the output shaft to 90 ft lbs. Is that the thing to do on a used setup? Also I had to install my bearing retainer housing on the used combo. After tightening the crush nut at the e brake drum, the drum studs would hold about 3 lbs of pennies (no fish scale) without spinning the drum...Your opinion on the 90 ft lbs at both locations and also the amount of output bearing preload? Thanks for the advice.
 
The torque on the bolts has no influence on the wear on the shaft. You need a new mainshaft in the tranny and likely a new input gear. If it's torn down already, now is the time to address this.

Seriously, though, what you have now is not acceptable.
 
those splines are just waiting to shear off of that shaft . buy a new one .
 
That transmission in the pics, and the t case with broken idler gear is not in the truck. (sitting on the ground, maybe rebuild for a spare) I bought a used 4 speed/transfercase from a 79 40 and put back in my truck. I knew from the looks of the splines I was not reinstalling that transmission.... The used combo that I put in my rig is the one I am questioning on the torque settings and preload. Sorry that I led you astray. Advice ?
