Why aren't more people going with higher wheel offset?

Yeah, it's me with another wheel post. ;) I was just curious why no one has had any of the wheel manufacturers do custom offsets for them? It seems like the main complaint about the Vossens is that they poke out too much. I know standard offsets go up to +30, but they can do any offset you want. Is there any reason not to have them do say a +40 on a 9.5" wheel? I think that would sit just about perfectly.
Also, on a slightly different note, what's everyone's thoughts on going with a 9.5" vs 9" wheel? I plan on going with 275/60/20 tires. I know the narrower the wheel, the more the tire looks like it's bulging a little, but is there any other reason to go with one size vs. the other?
