I can’t seem to find this blue and white label anywhere. Anybody have an idea where I can secure pictured item (warning label)?

I thank you, I’ve been on all those sites already and have order a couple others but all the diagrams that I have seen don’t actually show the wording, so I don’t know which one to get. The diagrams are so vague and I havent found a clear enough picture of what the hell it’s warning me about! If I knew that I would be a lot easier to locate.
