Need expert opinion on 200 sway bar ef up

I had installed an IM lift kit installed in dec. I am not sure what went wrong but sway bar had some how come disconnected. So a few questions

What part is missing? How do I fix it? How the eff did this happen?
 
The nut came off the end of the link bolt, that allowed the sway bar arm to drop. A new bolt and nut will be required, then lift the arm back up. It will be under much pressure, so you may need to carefully jack it up to position. The valve will need to be reset. Cause - bolt nut undertorqued.
 
can you take another picture of the linkage that goes from the lower control arm to the sway bar. in the first picture, it looks like the bolt and nut are on the linkage...just not connected to the sway bar.
also there looks to be tire marks on the sway bar, could the installer have somehow not put the bolt back through the sway bar?????

did the truck drive alright since dec, or has it recently gotten that way?
 
Yes. Looks like tire scrub on the end of the sway bar arm.
