It was the actual ECU/PCM. I replaced the crank position sensor first (similar symptoms and way cheaper) but ultimately it was the ECU. A key indicator is that when the ignition is turned on, the check engine light never comes on with the other warning lights and the RPM gauge never moves while cranking.
