Grand Caravan Ongoing Brake Issues

My wife has a 2003 Grand Caravan with about 56k miles on it. It has been having issues with the brakes intermittently since around 25-30k. Sometimes, but not every time, when we apply the brakes, there is a load groaning/grinding noise/sensation. It usually occurs during the last few feet of braking, right before the vehicle stops. I have replaced the front brake pads/rotors/ABS sensors and it seemed to be symptom free for a few thousand miles, but it did resume. Since it's my wife's vehicle and I try to avoid driving it whenever possible, I'm not entirely sure about when it resumed. Today, I had the rear pads replaced and they also turned the drums. The shop could find no other issues with the braking system except that the front pads showed 50% wear with under 10k miles on them. They also replaced the front sway bar bushings. The problem isn't as obnoxious, but it's still there. It doesn't do it every time I apply the brakes and, as best as I can tell, is unrelated to speed or braking distance. Can anyone offer any suggestions?

Not sure. I bought them through Autozone last summer. Was/is there an issue with rotors from China?
 
the chinese rotors warp easily and if you match them with ceramic pads it's accentuates it even more. do you tow alot or have a lot of hills or mountains there?
