File a complaint here @ NHTSA and get a campaign started.
Apparently Toyota is not going to step up and fix the issues unless they have enough complaints and the government feels that there are enough incidents to pose as a safety concern.

My best recommendation to determine if your frame is actually rusting from the inside out is to probe inside the access holes (both sides) that are found along the frame rail and/or run a compressed air line inside to see if any big chunks of rust come flying out.
My frame looked totally fine until I further examined it. Major scaling on the inside. Outside looks perfect. Well, it seems that the inside rotted so bad that my inner frame rail on the passenger side near the rear track arm had almost rotted through. It is soft like a sponge, but yet the factory Toyota frame paint held it together. Poking it with my finger and it all crumbled revealing quite a few holes.
 
Beat it with a rubber hammer. Any large scale that is built up should fall off with the vibration. Iron termites suck.
 
Unfortunately, with the boxed frame you will have to blow out the inner frame rails to get all that rust out. Once clean, to SLOW DOWN the rusting, get a garden sprayer and spray inner rail or any other rusting area with "Klean Strip" Prep & Etch". It has phosphoric acid which will turn the iron oxide aka RUST to zinc phosphate. Than it would be best to spray prime and paint the inner rails and other areas treated for rust. Ideally Penetrol can be used. I am doing the Eastwood inner frame spray along with etching primer and a final topcoate of paint. Once that has cured I'll be spraying most everything with Penetrol.
HOPE THIS GETS ME THROUGH THE NEXT 5 YEARS....LOL
 
What year 4Runner is this and where do you live? The frame on my 00 is solid as can be and it's a New England truck that honestly doesn't get washed much in the winter. My last 05 tacoma had a frame that looked like the titanic after 2 winters. My current 05 tacoma is really clean and I thoroughly painted the frame with eastwood rust encapsulator when I bought it last year. It's still really clean.
