Engine skip when warm...no codes

Our 99 with 252,000 miles has started skipping/misfiring when under load, but it seems to be only once it's warmed up. I thought it was a coil pack starting to fail, but that has not happened yet. Problem does seem to be getting worse. I've checked it twice and it's showing no codes at all. Through researching threads best i can come up with is a failing injector. Throttle body is clean, plugs have +/- 50,000 miles on them (possible bad plug too). all else is perfect. i don't know what else to check for before taking to local dealer... Thoughts?
 
MAF sensor? Vacuum leak? Bad gas? Could still be a coil pack - when one of mine started going wasn't missing enough to always throw a code but you could feel it.

If it turns out to be an injector - let me know how you diagnose. Have 241k on mine and while no symptoms yet - I wonder if old injectors are potential problem and if they contribute to lower mpg.

Wishing you good luck - finding the gremlins sucks sometimes.
