Clunk Noise on 2000 LC when shifting gears

I've been hearing a clunk(thud) noise on my 2000 LC when shifting gears from Park to Drive or Reverse, its aggravated when im on a slope, or an uneven road surface. If the vehicle is in motion I can also feel a slight jerk...also happens when I press my foot on the gas after I brake (both when I'm at a complete stop or in motion).

My thoughts are it could be the transmission mounts...but I'm no expert.

Have any of you experienced this?
The cruiser currently has 190k miles, regular maintenance has been kept upto date.
 
Is it in the front or rear? The front can have the biggest clunk. It is due to wornout CV axles and flanges at the end of the CV axle. How are the U-joints?

I do not recommend greasing the slip Yoke too much because the pressure created at compression (going over humps) will add excessive weight on the transfercase housing and it can crack. I usually add a one or two bumps from the grease gun for the front and rear drive shaft slip yoke. If you accidently added more, remove the grease fitting and bump the truck several times to push out the excess grease. Yes, use Moly grease for the slip yoke.
