It sounds like the transmission. I would start by checking the filter isn't blocked creating pressure problems. Otherwise it's not going to be a cheap fix.
Did they check the bearings for wear? Loose bearings don't make much noise, but worn/dry bearings get quite a grumble.
If it's 200k miles on original bearings (quite possible) then it could just be coincidence, and not accident related.
