Just to update on the original problem, it was indeed a fuel injector.
Had them all bench tested and adjusted to the FSM specs. Did not have to replace the points. New washers were installed and for safe measures new fuel filter.
Runs very smooth, more power and sounds more quite.

Yes, the diesel here in Central America is 1 part diesel and 2 parts dirt/water. We all run a pre filter but still change the fuel filters more often than up north.

Materials:
Liqui moly purge (used due to the good reviews), 3' rubber hose, mason jar or any clear container. In line fuel filter if you have a return fuel line. Couple of beers wile you sit around waiting...

Thanks Tom. After driving these diesels long enough I feel you can tell when a noise occurs that will indicate something is not right. I am half tempted to do a diesel purge before dropping the $$ on rebuilt injectors. It's a long shot but with the horrible diesel we get in Costa Rica maybe...
Figured it out. The #4 injector is bad. I switched #3 in its place and the sound defiantly decreased. So, I will have the local shop rebuild all 4 for me now. Sure to experience more power and fuel economy. No telling how old these injectors are. Thanks for all the input.

I took off the valve cover and checked all the clearances. All good. Next, I removed the #4 cylinder injector and the noise seemed to decrease a lot. Since I could hear in the cab I decided that maybe this injector is clogged or possible bad. Going to remove this injector and see if that is...
I thought if it was a a loose exhaust valve slap I for sure would hear it in the engine bay around the valve cover. Given how loud the sound is in the cab it would be much worse at the valve cover???
When the engine is warm, the Engine bay is much More quite but still that loud sound is coming from inside the cab. Mainly at idle and slow acceleration.
