Help with Serpentine Belt issue

So a few weeks back I noticed that the serp. belt was off track, I had trouble with the tensioner so I replaced it with the new model and a oem belt, it worked fine. This morning my wife calls me to tell me the battery light is on and No power steering. She found the belt came off.

I am sure that both incidents happened after using the A/C. She also send me a pic of the power steering reservoir leaking a bit near the cap...not sure if that is related.

A/C worked fine, is it the pully that needs attention? Any suggestions as to what the problem might be?

If the power steering reservoir is between the Min and max range and you turn the wheel more than 1/2-3/4 of a turn, it will overflow when the pump is not running. You need to take a close look at the alignment of all the pulleys; you can check alignment with any type of straight edge. If that is ok, check for not so obvious bends in the pulleys.

Questions: Are you sure it is the correct serpentine belt?
You say you had trouble with the tensioner. Was your belt routing correct?

Really, since the original belt popped off and didn't break, I'd say the best bet is in a misalignment issue. Remember that there is a reason something became misaligned, so find out why that is.

I would check all of the rotating units like the idler pulley, fan, power steering pump, alternator, A/C pump, to make sure all of them spin smooth and freely. If any are binding or grabbing as they spin it might cause the belt to jump off as the tensioner arm gets jerked and then snaps back.

Yes, do this first. I assumed it was something that would have been done when the first belt was replaced, but you know what they say about assuming.
 
The belt I used is oem, im looking for the receipt to check the part number. I will check all pully when I get off work. Thanks guys!
