95t100 rear disc brakes

Im looking to do a disc swap on my 95t100. If this is in the wrong area feel free to move it. the diff is the 8'' 2 pinion that is also found in 00 to 05 tundras and some 4x2 v6 tacomas. Im gunna use stock ifs front calipers and get a proportioning valve. is anyone aware of where i can pick up a set of disc mounts for the axle? some info would be great. or a point in the right direction. thanks for your time

and would i be able to use toyota calipers? ive seen kits that use chevy calipers. but none for toyota calipers.
