Ok I'm going to ask because I don't know. So in the pictures I've seen LHD and a RHD. Close up of the shifter shows that all the gears are in the same location but the shifter leans towards the steering wheel weather it be LHD or RHD.I guess I thought in a RHD vehicle 1st gear would be the gear farthest right and on top. Anyone help me out here?
 
The gear pattern is identical whether it's LHD or RHD. First is leftmost and up, reverse is rightmost and down. The shifter does lean towards the driver, otherwise it would be a longer reach.

I think I would prefer having first closest to the driver, but I've gotten so used to it that I haven't really noticed for years. If they made it mirrored, then they would have to have 2 versions of every manual transmission.

Yes.
The "pattern" would be identical whether LHD or RHD ir you were to look down on the shifter from directly above (where the sunroof is), same with the pedals on the floor, clutch to the left, brake in the middle, and the skinny pedal on the right.

Here's my info:

8. Nothing yet, but have a "short-throw" kit that I got in Japan

10. 18 mpg(US) around town. 20-ish if driven at 60mph on the freeway. One time I've gotten something crazy like 26 or 28mpg when I was in Moab and drove to Arches NP, etc... doing mostly 40-45mph and coasting downhill, etc...
11. Stiffer unti-awaybars (removed to fix the broken brackets)

That's a nice clean looking truck! I wish my paint still looked that good.
