From the attached TSB on exhaust booming- note the direction of the transmission mount when re-installing:

Just fixed the vibration in mine.

I found the TSB on Exhaust Boom at Idle posted by another member (Attached).

After going through everything listed, mine was motor & transmission mounts. Installed (2) 12361-50210, and (1) 12371-50180. It runs smoother than it has in years. Noticeably smoother at idle, and at 75 MPH.

Used SLEE SliderSteps, instead of the (2) OEM cross member dampers listed in the TSB.
For the record, the 1998 model year part numbers are (2) 12361-50210, and (1) 12371-50080.
 
If I remember correctly one of the bolts that fastens the replaceable rubber mount to the frame mount has a captured nut- that would be the forward bolt (front of engine). In case youre stuck trying to figure out how to get that nut off from the bottom.
 
Replaced the trans mount today and the idler pulley after work, ran out of time for engine mounts. There was a wider gap on the trans mount where the bolt thats tack welded goes through the middle of the trans mount, i dont know what to make of it? Do any of you know what the torque setting for the idler pulley is? I just got it tight as i could. Then a little more.

Dont do anything with the center bolt that goes through the trans mount.

I think the idler pulley torque spec is 27-29ft lbs
 
I didnt touch it, i just thought it was odd that the gap was different. Im sure it has to do with getting it into spec, prob not the same on every one. Cool i set the idler pulley at 30 ftpounds or 360 in on my cheap wrench. My old idler pulley was on it way out, got a little catch in it. @abuck99 i think i understand about the nut. Question: do i unbolt the 2 bolts from frame first then lift and get the 1 bolt from mount to engine off or the other way around? Or does it even matter?
 
Re; engine mount: I think I removed the nut on the rubber mount first then the two bolts that fasten it to the frame mount. Then lift, remove and replace.
 
Done!!! Here are pics of my old mounts, you can see separation from the metal all the way around. 2003 with 145k miles, vehicle lived its whole life in florida and Texas. Found out ive got a leaking cv shaft. Always something. See pics of mount. Havent driven to see any changes. Drivers side was the worst!

Wanted to update. Motor mounts and transmission mounts made my truck run smoother than before, especially at highway speed. I am still experiencing a drone though, although not as bad as before. Not sure what to do next? My thoughts on this have gone back and forth and I believe the frame of the truck transmits vibrations into the cab in drive, I do not think there is any correlation to exhaust. I do not currently have sliders but thinking of purchasing some with the thought that they would in some way help displace the vibrations.
One thing I have not tried yet is that frame weight thing, I just don't see how that would make a bit of difference? I really wish being anal wasn't in my DNA and I could live with things. Any suggestions appreciated.

Work your way through the TSB for the exhaust boom, I’ll bet you find it in one of the items listed. Yours may require all the items listed.

Yeah ive been reading through it for a couple hours. I doubt im gonna replace the rear tailpipe, thats just stupid. After looking at the tsb working through the exhaust with new nuts and gaskets seem like something i could try if i could figure out part numbers(the tsb doesnt show part numbers). I already have a dampner on my exhaust pipe, i guess from the factory. Ive done engine and trans mount so i dont need that. I dont want to do the weight thing cause im thinking about adding sliders anyway.
 
I'd bet its exhaust related. Had a 2002 RX which had the same symptoms and we ended up finding a 1/2 cm leak in the exhaust, patching it promptly restored the Lexus silky smoothness.

Also, I don't believe the TSB called for engine mounts. I may be wrong, but it seemed like transmission mount only.

Yes it does call for replacement of trans and motor(its repair procedure g and h). Im on to exhaust now, gonna try to figure out part numbers for gaskets and nuts and work from the front to the back. It just doesnt make since why it doesnt drone in neutral or park if its exhaust related.
 
Sliders wont really make a difference in what youre experiencing. The frame has a resonance at certain rpms from a combination of engine vibration and exhaust- thats where the TSB stemmed from. Ive played around with adding some exhaust dampers in various spots along the exhaust system with some improvement but its not solved completely. Not that big a deal really, but if you come across one of the frame weight isolators, that might help.

Ive got a guy on here that has one he will sell me. Heres what i dont know:
Are there different styles of these weight isolators? They way i read the TSB it wants you to put one on driver side then possibly the passenger side.
 
IDK, Ive got sliders also have full skids and steel on both ends, all connected to the frame - its pretty ridgid prob more of a tuning fork than before all the extra armor.
 
I replaced muffler with Magnaflow and removed resonator. 90% of vibration disappeared. Changed motor and trans mounts and now free of any vibrations. Took weight off and didn’t notice any difference.
