Anyone have a clue?

Small coax like cable is what is connected. Found when doing stereo work near stock amp under dash.

The ~pl-259-ish and coax suggests antenna, the "do not rotate" warning suggests directional, the small size suggests uhf or microwave.

Garage door opener??

It sure looks like a coax connector but not a PL-259 which is much larger. Garage door openers use a frequency range somewhere about 325 Mhz. Somewhere on the device there should be an FCC-ID that can be looked up on the FCC website or a model number that can be googled.
 
My guess is an antenna for one of the old cellular 'bag' phones. Those had a smaller coax connector.

Or maybe a LoJack antenna?
