Yeah, that's a place my mind went, as well. Be he's a well known guy, and I've watched him be very methodical doing other work, including a swap and partial rebuild of a 1FZFE. I guess I'll never know.

If he's a reputable mechanic, I'd be involving him in any steps going forward. He's got to know basically everything he touched fell apart and let him decide how if he wants to assist in any way, his reputation and pride in workmanship should be worth something.

This is a rough situation.

I wonder, if the new pulley purchase is documented, is there any comeback on koyo?
The bearing should be fit for purpose, purpose being minimum of 100k km service interval. I don't think the FSM calls for the pulley to be changed with the T belt, so its service life is theoretically lifetime of the engine
 
Is the bolt holding the idler pulley loose? Did the bearing fail first, or did the bolt loosen causing the bearing to fail?
 
Doubt the bearing failed without underlying cause. Could it be a Chinese knockoff?

That's the culprit. Without that bolt the tensioning mechanism wouldn't work. Its hard to say whether it was forgotten , or fell out. My guess it was forgotten. I cant understand why it wasn't seen, the belt should be quite tight once its on correctly.

maybe he try to set the spring then the bolt ( impossible mission ) ..?

I can't see it having been forgotten, as I ran for 50K KM, since that was done. Maybe it backed out.
 
Its pretty well sealed with the plastic cover, unless you noticed gaps when you were taking it off maybe from the idler pushing on it and the bolt fell out, otherwise it be pretty hard to miss that unless it was forgotten. On the other hand without the bolt, there is no way the idler could keep the tension, so running it 50k like that is fairly impossible, conclusion, the bolt was there and it backed out, look closer its somewhere in there.
 
This thread gets me thinking that I got to get crackalacking on my timing belt replacement, tensioner, and H2O pump service. Wishing you nothing but karma and a prayer.
 
Be cool for one of you more adept 1hdt guys to do a walk through for the average owner on how to pull the cover and do an inspection and perhaps locktight this and that as a preventative maintenance item. Its such a nice motor to be damaged by a stupid thing like a bolt backing out.

If properly torqued it's not coming out. I wouldn't think of this as the kind of thing that needs to be checked on outside of the regular maintenance interval assuming it was installed correctly.
