I have only recently come to the realization that there is a place for rear seat grab handles built into my 85 4Runner (see photo). My 86 was an SR5 but I can't remember if it had them or not (but I assume so). I have researched installing a front grab handle on the driver's side and have one from a parts truck on the way. It appears that the front and rear are interchangable (albeit with different mounting screws apparently) and Roger Brown's website (4Crawler) documents moving one from the rear to the front so that looks like it works without problem.

I have measured the various grab handles in my 100 series (DD in England) and of the 10 that are installed, it looks like the A pillar ones would work very well in the 4Runner front or rear.

Finally my question: Has anyone installed grab handles from other generation 4Runners and/or Land Cruisers, etc. either front or rear and, if so, what worked and what didn't?

Yep, though I used handles from the same model years. There is only a handle on the passanger side door, IIRC. You can install the same handle in the back seat on the shell, and the holes pre-exist above the drivers door for a handle too.
