May seem like a strange question but does the smoke smell sweet? Burning coolant has a sweet smell. I suggest a compression test.

How is your coolant suited to the cold weather? Any chance of the block freezing?

I wish I could tell. I did smell it but I obviously have no idea what to compare it to. It smells nasty, not like typical exhaust if that makes any sense.
 
Have you checked plug wires are routed correctly ? Seems simple but worth a look...

Just saw 2nd part of your question. I don't think it's possible for block to have frozen. Coolant is appropriate and truck is kept in a garage that I have heaters in (for the dog) when cold...
