Looks like I solved the issue. Drove 3000mi over the holidays and had zero CEL/MIL light. Ultimately the problem was a break in the VGND (black) wire caused by corrosion secondary to water/dust intrusion into floor wiring harness #3.

Spoke too soon. VGND had a break in continuity somewhere, so I just ordered a whole new harness. Hopefully that's the end of this nightmare.
I think I figured out the problem. One of the male copper parts in the harness broke off inside of the female part of harness. Looked at EWD and it's the "VGND" that connects pump module to ECM.

Well I replaced filter, and now the problem is worse haha. Use to be 50/50 on P2420 code on start up and 1 out of 10 times getting a P0441, now I get it 100% of the time after x2 cold starts, or mid drive. cracked open the old filter, some dust on the clean side but nothing crazy.
