Ya, under the dizzy gear on the end of the cam is another bolt, usually hiding in a puddle of oil. 12mm head.

Yup, got that one. I don't see anything else holding it on...

...big hammer?

Resolved!

Turns out all the bolts were out. The head was just stuck. Ended up using my engine hoist to provide tension then hit the top of the head with a dead blow hammer. After a few whacks, the head popped up about 5 inches! Maybe too much tension....
