My guess is that cylinder running lean. Maybe swap the injectors around and see if a different plug ends up like that. I'm guessing it has some build up and isn't spraying as expected.

That's a solid theory.
Or it could be a fault in the turbo encabulators coptic marzel vane, which of course is a result of excessive fluxes and magnito reluctance. However, that would only be applicable on days of the week falling on odd and/or even numbers.
 
I think more people should be giving a serious look at injector cleaning as they rack up miles. We do have a pickup screen, but without a real fuel filter there are a lot of opportunities for junk to make it to one or more of the injectors. Looking at LTFTs can give an idea something is going on, but in theory if one is clogged on each bank it would be easy to believe the increase is just a function of things wearing with mileage.

I don't know what fuel was put into my rig for the first 100k, but I generally use well-trafficked stations. Plus when I dropped the tank to change the pump as PM at ~200k the inside of it was shockingly clean. Still, my injectors reported some pretty bad numbers when I had them cleaned as part of the HG job. In my opinion, at least.

No idea if that's what's going on here, but there's a chance.
 
I don’t think the plug is quite showing a lean condition, but it could be.

I have no idea what fuel was run for the first 160K miles but I only use top-tier about half of the time. I do add a can of BG 44K to the tank at about every other oil change, but that’s only been maybe 3 or 4 times. Maybe I’ll yank these injectors and send them out for cleaning. Maybe not 🧐
 
I try and run a bottle of fuel cleaner such as the Amsoil stuff or Techron every few thousand miles or so. The last I did a few days ago at 80,000 miles, was the Amsoil cleaner. I then hooked up my scanner to see what the live data would say. The fuel trim for both banks was 0.0, right where it should be, indicating the injectors are clean and the computer is not adding or subtracting fuel to compensate. For what ever this may be worth...
