It's not obvious but there are two versions of the BFG KO2.. 3 peak snowflake (3PMSF) version with no mileage warranty, and the non-snowflake "DT" version that has a 50K mileage warranty. BFG website doesn't mention two versions. It says KO2's have a 6 year warranty, and a footnote reads...
Do you know what type of oil was in it? brand, synthetic vs conventional? I'm also curious what the properties were on the lower section of the report where it got cut off. 5K mile OCI is a little long IMO. Besides contaminants being high the additive package is a bit on the low side.

When I took the air cleaner and intake tube off to get at the plug wires/holes it was clean.

I searched and didn't see any similar threads. This is my 1996 with 130K miles and I've owned it since '07. It's never had any problems, not head gaskets, not PHH, non of the often cited issues. Oil has been changed every 3K miles and last change the UOA was fine.

If the battery is fully charged it would be 12.7 volts at rest not 12.4 -- 12.4 would be around 70% state of charge for a good battery. If it ever goes to 9-10 volts it's completely dead. Further, if it was a good battery, the current needed to pull it so low would more than likely smoke a 10...
I don't have a stock dash or gauges, so I'm not sure. One thing for certain there's very little space between the gauge cluster and firewall. I think 6" of cable plus VSS would be a tight fit.

I would guess above 2800-3000. You probably wouldn't notice a difference. The ECM should have a sticker on it with a code. A web search should bring up more info about what engine it was originally for, e.g. 305 or 350, and programming.

10 micron filter on the supply side of the pump may be too much restriction. You may want to check. I would put that Racor between the pump and engine and a 300-400 micron between tank and pump.
